Due to the existence of the current "window period" of the virus, the limitations of detection techniques, the emergence of new viruses and the existence of unknown viruses, blood-borne diseases cannot be completely eliminated. Therefore, even blood that has undergone strict routine testing may present a risk and probability of viral infection. In addition, blood transfusion may also have adverse reactions such as allergies, hemolysis, fever, ineffective platelet transfusion, acute lung injury, and graft-versus-host disease associated with blood transfusion. In 2019, in order to standardize clinical blood transfusion medical behavior, we minimize the risk of blood transfusion. For the maximum guarantee of medical safety, we have done the following work:

First, establish and improve the hospital clinical blood transfusion management institutions

In order to ensure the safety of clinical blood, our hospital has established a blood transfusion management committee consisting of the director of the dean, the head of the medical department, the head of the laboratory and the director of the relevant clinical department. According to the "Administrative Measures for Clinical Blood Use in Medical Institutions", "Clinical The Technical Specification for Blood Transfusion is strictly in accordance with the Regulations on the Administration of Blood for First Aid in Obstetrics, and provides technical guidance and supervision for clinical blood transfusion work, guiding the rational use of clinical blood, blood components and blood products, and coordinating the major problems encountered in clinical blood transfusion work. The problem is to ensure the safety of clinical blood transfusion.

Second, strengthen the training of medical personnel blood transfusion regulations knowledge

Medical staff not only need to master their own professional knowledge, but also should be familiar with the laws and regulations related to blood transfusion. Clinicians should pay sufficient attention to relevant regulations in order to truly achieve legal, scientific, reasonable and safe blood transfusion. If you do not understand the blood transfusion related laws and blood transfusion regulations, it may cause mistakes in your work, leading to medical disputes. Therefore, we have conducted relevant trainings on relevant laws and regulations. At the same time, our hospital has also sent 3 batches of 8 people to participate in the “Professional Training of Clinical Blood Collection and Supply Organizations” organized by the Provincial Health Department. The training includes current and blood transfusion. Relevant laws and regulations: "The Blood Donation Law of the People's Republic of China", "Administrative Measures for Clinical Blood Use in Medical Institutions", "Technical Specifications for Clinical Blood Transfusion" and new technologies related to improving blood transfusion safety.

Third, strengthen the supervision and management of clinical blood use

Irregularly check the writing status of the “blood transfusion application form” and the blood sample collection for each clinical department; check whether the “blood transfusion application form” meets the requirements for blood transfusion, whether the blood volume has been approved or signed, and the monthly report, annual report, etc. Statistical report, Juguan masters the use of blood and reasonable and scientific blood use; Randomly check the medical records of transfusion treatment cases submitted by various clinical departments, check whether there is a "transfusion consent form" and make a record.

4. Supervising the daily business of the blood transfusion department and promoting the promotion and application of new blood transfusion technologies

With the rapid development of transfusion medicine, the concept of component blood transfusion has been deeply rooted in the hearts of the people. Many new technologies related to improving blood transfusion safety are used in clinical treatment, which plays an important role in effectively reducing the incidence of adverse reactions of transfusion in patients. Irregularly check whether the various registration records of the Department of Transfusion are complete, whether the archive storage is complete, clear, and ready for future reference. The blood storage management is checked from time to time to ensure the safety of clinical blood. Blood was used for 41 people throughout the year, and blood transfusion cases were consistent with blood transfusion indications. Among them, the red blood cell suspension totaled 131 units, the plasma was 1550 ml, the whole blood was 800 ml, and the component blood transfusion rate was 97.2%. There were no medical disputes caused by clinical blood transfusions throughout the year.
