Class teacher, student and parent safety education responsibility book

Your parents:

Your child is studying at our school---grade. In order to ensure your child's personal safety and make your family happy, we have made this responsibility book to attract the attention of parents and students, and work with parents to do safety and prevention work. Eliminate the occurrence of student injury accidents.

1. On campus:

1. After arriving at the school, no one can go outside the school to play outside the school time. If there are special circumstances, you must give the teacher a leave of absence, write a leave of absence, and go out with the teacher's permission.

2. During play, you should not push, pull, hold, squeeze, chase, and do not play unsafe games. The stairs up and down should be on the right. Otherwise, the school will not bear any responsibility.

3. In the physical education class, the training should be carried out under the guidance of the teacher, and no private activities should be carried out. Otherwise, the teacher will not bear any responsibility for the accident.

4. The upper and lower stairs can't run, and they can't climb down the handrails of the stairs. Otherwise, the class teacher will not bear any responsibility.

2. Outside the school:

1. Do not play with fire, do not play with electricity, do not play with water, do not play in dangerous, remote places, and do not play on the road.

2. Ride a bicycle on the road or on the street.

3. If you have something to go out, you must greet your parents and you cannot stay outside.

4. Do not buy unhygienic diet to prevent poisoning.

5. Improve self-defense awareness and protection to prevent the temptation and abduction of the bad guys. The above nine requirements require that each student must do it, otherwise they are responsible for their own responsibility. I hope that the parents will cooperate with the school education students to consciously abide by.

This letter of responsibility shall be effective upon signature by the class teacher and the parent, and shall be signed once every academic year.
