1. The birth certificate certificate and related materials for newborns born in normal hospitals:

1. Determine the name of the newborn.

2. Provide the following materials: birth medical records; birth certificate application form; original and copy of both parents' ID cards; reproductive health certificate; maternal health book.

3. Time for accreditation: Monday, Wednesday and Friday.

4. Others: The birth medical certificate is a legal certificate. Once processed, the name of the newborn cannot be changed, and any one of the newborn parents is required to handle it personally.

If you have more than 3 months after birth, you must also provide proof of residence registration or residence book for both parents.

Second, the family birth certificate for birth certificates:

1. For the newborns who receive the birth certificate of the birth attendant's technical qualification certificate, the birth attendant of the family shall issue a birth medical record containing the relevant contents of the birth medical certificate, and go to the local maternal and child health care institution to issue the biomedical certificate.

2. For newborns who are not authorized to carry out midwifery technical services and those who have not obtained the family birth certificate technical qualification certificate, go to the local maternal and child health care institution to issue a biomedical certificate. Ask the newborn parent or guardian to issue the following circumstantial materials:

A “parent-child relationship statement” issued by a newborn parent or guardian.

The circumstantial material of the parental relationship between the newborn and its parent or guardian: refers to the birth medical record issued by the birthing unit or individual, the certificate issued by the resident committee, township government or unit where the parent or guardian of the newborn parent is registered, or the paternity test certificate.

The reissue method is as follows:

If the "Birth Medical Certificate" is lost before the account is reported, the "Date of Health Certificate" is reissued;

If you have lost the "Birth Medical Certificate" after you have completed the household registration procedure, you will only reissue the "Birth Medical Certificate".

The replacement of the "Birth Medical Certificate" applies only to babies born after January 1, 1996.
