Second, the assessment object and time limit

The object of assessment is the licensed physician and practicing assistant physician who have obtained the qualification of the doctor according to law and have been registered in the hospital. The physician categories are clinical, Chinese medicine, oral and public health.

Physicians regularly assess each cycle for two years. During the assessment period, the physician who intends to change the place of practice or who has one of the conditions listed in Article 37 of the Practitioners Law but has not revoked the practice certificate shall conduct the assessment in advance. The doctor's assessment that he did not participate in the assessment without proper reasons during the assessment period was unqualified.

Each year, the doctors who have registered for two years of examination will be assessed. Generally, they will start in December of this year and complete by the end of February of the following year. Before the end of March, the “Registration Form for Regular Assessment of Physicians” will be submitted to the registration authority. The assessment results are notified in writing to the person. In the assessment cycle, if the assessment fails, it needs to be re-evaluated. Examination content

The regular assessment of physicians is divided into three parts: job performance assessment, professional ethics assessment and business level assessment. The performance appraisal content includes: in the process of practicing the doctor, complying with relevant regulations and requirements, and completing the work quantity, quality and government's mandatory work during the appraisal period.

The content of professional ethics includes: the persistence of life-saving and ambulance in the practice of doctors, the "patient-centered" service concept, medical ethics, doctor-patient relationship, solidarity and cooperation, legal practice, and the Ministry of Health's guidance on establishing medical ethics assessment system for medical staff The provisions of the Opinions, etc.

The business level assessment includes: the business level assessment is based on the level, category and professional treatment of the licensed doctors, focusing on the evaluation of laws and regulations and the “three basics”, as well as the ability to learn and master new knowledge, new theories, new technologies and new methods. The business level measurement score is divided into two parts: clinical basic skill operation assessment and health regulations and professional knowledge comprehensive written examination.

Assessment criteria:

1. Work performance and professional ethics

It is combined with the appraisal doctor's assessment of the previous year and the usual assessment and evaluation results.

Establish a sound work performance and professional ethics evaluation system as a basis for professional ethics assessment of physicians.

The work performance and professional ethics assessment shall be submitted by the department where the appraisal physician is located to submit assessment opinions and reported to the Personnel Section for review.

2. Business level assessment

Business level assessment can take the following forms:

Fourth, the assessment program and method

Before the implementation of the assessment

1. The appraisal office will notify the doctors who need to be regularly assessed on the assessment date 60 days before the assessment date, and report to the physician registration authority.

2. Within 10 days of receiving the above report, the physician registration authority will provide the examination office with the administrative punishment within the examination period of the doctor within the list.

Assessment implementation

The assessment program includes simple programs and general programs.

1, simple program

Has more than 5 years of practice experience, and has a good track record during the assessment period;

Has more than 12 years of experience in practice, no record of bad behavior during the assessment period;

Those who have deputy chief physicians and above technical positions have no bad behavior records during the assessment period;

After the doctor retires, he is rehired by the unit and there is no record of bad behavior during the assessment period.

For physicians who meet the simple program assessment, I will write a report on the work in terms of work performance, professional ethics and business level. The department will sign the assessment opinion on the "Physician's Periodic Assessment Form", which will be reviewed by the Personnel Section. The assessment office approves and approves.

2. For physicians who adopt general program assessment, they first write a report on their work in terms of work performance, professional ethics and business level. The department conducts work performance and professional ethics assessment, and signs it on the Physician Periodic Assessment Form. The opinions shall be assessed and reported to the Personnel Section for review on the 30th day before the business level assessment; after the review is completed, the assessment committee office shall organize the evaluation of the business level.

After the implementation of the assessment

1. Within 30 days after the assessment of the appraisal office, the “Registration Form for Regular Assessment of Physicians” and the “Registration Form for Physicians” will be sent to the evaluation committee and the physician will be notified. At the same time, the “Registration Form for Regular Assessment of Physicians” will be reported to the health supervisor as required. The department has filed the record.

2. If the assessed physician has any objection to the assessment result, he/she may submit a review application to the assessment office within 30 days of receiving the assessment result. The appraisal office shall organize a review of the doctor's assessment results within 30 days from the date of receiving the application for review, and notify the physician in writing of the assessment opinions.

V. Practice behavior and record of assessment results

The appraisal office shall establish a regular appraisal file for the appraisal physician, fill in the form and file description according to the “Physician's regular appraisal file” to establish a physician file, and deposit the physician practice record and the “Physician Periodic Appraisal Form” into its file in time. The establishment of the file and the daily management are the responsibility of the hospital medical department.

Good behavior records include doctors receiving awards and recognitions at the county level or above in the course of practicing, completing government-designated tasks and technical achievements, etc.; bad behavior records include administrative penalties for violations of medical and health management laws and regulations, and medical treatment norms and routines. , disciplinary action, and medical incidents that occur.

The results of regular assessments of physicians are classified as qualified and unqualified. Any one of the work performance, professional ethics and business level that cannot be assessed or evaluated, or one of the situations listed in Article 27 of the Physician's Periodic Assessment Management Measures shall be unqualified.

Within 30 days after the end of each assessment, the hospital assessment office will promptly submit the “Physician's Practice Certificate” of the assessed physician to the registration authority. If the assessment is unqualified, the assessment will be carried out again after 3-6 months of training. If the assessment is still unqualified, the health administrative department will cancel its practice registration and withdraw its "Physician Practice Certificate".

7. Supervision and management

The regular assessment committee of the hospital physician is responsible for supervising the assessment work of the assessment office. During the one-week examination of the assessment office, the materials regularly reviewed by the physician are randomly selected. Violation of the "Administrative Measures for Regular Assessment of Physicians" by the appraisal office will be submitted to the public in accordance with the relevant provisions of Articles 28 to 31 of the Practitioners Law, the Regulations on the Administration of Medical Institutions and the Measures for the Periodic Assessment of Physicians. It is strictly forbidden to falsify, abuse power and engage in malpractices during the assessment;

Anyone who fails to participate in the assessment within the stipulated time must submit a justifiable reason to the appraisal committee. The appraisal committee will arrange the time assessment separately. If there is no justifiable reason not to participate in the appraisal, or the order of the appraisal is disturbed, the appraisal cycle is unqualified.

Third, the assessment object

The basic content of the doctor's business level assessment should include: the doctors master the relevant laws, regulations, departmental rules and regulations of the medical and health management, and apply the basic theories, basic knowledge, basic skills of the major to solve practical problems, and learn and master new theories and new knowledge. Practitioners should be able to independently deal with common diseases, frequently-occurring diseases, general emergencies and common professional and technical problems in this profession; practicing assistant doctors can handle the above problems under the guidance of practicing doctors. V. Assessment implementation and management

Centralized assessment

The doctors regularly evaluate each period for two years. The evaluation of the business level is concentrated in the evaluation from October to November of the second year. The assessment committee should notify the doctors who need to receive regular assessment 60 days before the regular assessment date.

Implementation time

Before September 10, the medical department should summarize and document the basic information forms of the appraisers.

Before October 10, complete the doctor's work performance, professional ethics assessment, and sign the assessment opinions on the "Physician's Periodic Assessment Form".

Before November 10, complete the business level assessment.

Before November 20th, the doctors will make an assessment conclusion, and sign the opinions on the "Physician Periodic Assessment Form", enter the doctor's assessment results into the assessment management system, and submit the "Physician's Periodic Assessment Summary Form" to the Health Bureau of the Health Bureau. . Relevant materials shall be provided for the circumstances listed in Article 33 of the “Administrative Measures for Regular Assessment of Physicians”.

Before December 5, the “Physician's Practice Certificate” of the appraisal physician will be submitted to the Medical Affairs Section of the Health Bureau.

During the assessment period, if it is proposed to change the practice location or violate the provisions of the Law of the People's Republic of China on Practicing Physicians, one of the following circumstances shall be imposed by the health administrative department of the people's government at or above the county level, but the physician who has not revoked the practice certificate, The assessment should be conducted in advance. 8. Experimental clinical treatment of patients without the consent of the patient or their family members;

9. Leaking the privacy of patients and causing serious consequences;

11. In the event of natural disasters, epidemics of infectious diseases, sudden deaths and serious accidents, and other emergencies that seriously threaten people's lives and health, they are not subject to the dispatch of the health administrative department;

6. Practice record and assessment program

Implement a system of physician behavior records. Physician behavior records are classified into good behavioral records and bad behavioral records.

The physician's behavior record is one of the basis for the regular assessment of the doctor. The hospital establishes the doctor's regular assessment file, and deposits the doctor's practice record and the “Physician's regular assessment form” into its file in time.

The assessment program is divided into general programs and simple programs.

Seven, assessment results

The assessment results are divided into qualified and unqualified. Any one of the work performance, professional ethics and business level that cannot be assessed or evaluated is unqualified.

Physicians in the assessment cycle through the resident standardization training in accordance with the regulations or through the promotion of the upper level of professional and technical job examinations, can be regarded as qualified for business level assessment, only assessment of work performance and professional ethics.

For those who fail to pass the examination, the practice will be suspended for 3 months to 6 months, and training and continuing medical education will be accepted. After the suspension of the practice activities expires, the assessment committee will conduct another assessment. For those who pass the examination, they are allowed to continue practicing, but the doctor shall not be evaluated and promoted during the assessment period; if the assessment fails again, the health administrative department shall cancel the registration and withdraw the doctor's practice certificate.

The assessment committee comprehensively evaluates the opinions and the results of the business level assessment, makes the assessment conclusions for the doctors, signs the opinions on the “Physician Periodic Assessment Form”, and summarizes the doctor assessment results according to the unit, and fills in the summary form, after the end of the regular assessment work 30 The daily newspaper reported to the health administrative department for the record.

If the assessed physician disagrees with the assessment results, he may submit a review application to the assessment committee within 30 days from the date of receipt of the assessment results. If the physician has one of the following circumstances during the assessment period, the assessment committee shall determine that the assessment is unqualified:

3. Conducting practice activities across the practice category;

4. Participate in the physician qualification examination on behalf of others;

5. Asking patients, their relatives and friends for property or other improper benefits in medical and health service activities;

6. Ask for or accept the rebates, commissions or other improper benefits granted by the production or sales enterprises, medical products, reagents, etc., or their staff;

7. Receive rebates or commissions by introducing patients to other units for examination, treatment or purchase of drugs, medical devices, etc.;

8. Produce false medical certificate files and participate in false medical advertising and drug medical device promotion;

9. The hospital infection control task was not carried out in accordance with the regulations, and disinfection or harmless disposal was not effectively implemented, resulting in disease transmission and epidemic;

10. Deliberately leaking information and materials concerning the privacy of infected patients, pathogen carriers, suspected infectious diseases, and close contacts;

11. Failure to perform the duties of monitoring, reporting, investigating and handling infectious diseases in accordance with the law, causing serious consequences;

12. During the assessment period, more than one medical ethics evaluation result is poor medical ethics;

13. Do not participate in the assessment without proper reasons, or disturb the order of assessment;

14. Violation of the relevant provisions of the Practitioners Law, and being punished by the administration.

Eight, relevant requirements

Part 3: Physician regular assessment implementation plan

Before April 20th, each appraisal institution will send the "Registration Form for Regular Appraisal of Physicians" to the institution where the doctors are practising, and notify the appraisers in writing of the results of the appraisal. From April 26th to April 30th, the appraisal institutions will report the "Physician's Periodic Assessment Results Registration Form", the 20XX Annual Physician's Periodic Assessment Report, and the Physician's Periodic Assessment Form to the Health Supervision Office of the Municipal Health Bureau. 4. Business level assessment team:

3.
