The implementation of family planning is a basic national policy of our country. According to the "_______ Provincial Plan and Childbearing Regulations", "_______ Provincial Implementation Measures for Floating Population Planning and Family Planning" and the "_______ County People's Government's Implementation Measures on Implementing the Regulations on the Implementation of the <_______ Provincial Plan and Childbearing Regulations", Party A and Party B agree to sign this contract.

1. Party A’s obligations

1. Organize unmarried youths three times a year to plan national education for family planning and send them to the National Policy Education Certificate;
2. Assist Party B in handling the marriage registration formalities in a timely manner;
3. Organize Party B's pre-marital health check and provide consulting services such as new marriage knowledge, contraception and birth control, and prenatal and postnatal care;
4. Establish a family contact system for the Family Planning Association and visit it regularly;
5. Give spiritual or material rewards to young people who are conscious of late marriage and late childbearing.

Second, Party B’s obligations

1. Participate in the national policy of unmarried youth planning and birth three times a year on time;
2. Registering marriage according to law, not early marriage and early childbearing;
3, do not illegally cohabitation before marriage, if there is illegal cohabitation or introduction or abortion before marriage, participate in three times a year pregnancy check;
4. Voluntarily accept visits from leaders, cadres, and members of the Family Planning Association.

Third, liability for breach of contract

1. Party A does not conscientiously perform the education and supervision duties of the planned national policy of birth. If one of the following acts is committed, Party B must pay Party B a penalty of _____ yuan.

2. If Party B fails to perform its obligations as stipulated in the contract and has one of the following acts, Party B must pay Party A the liquidated damages, but the guarantor is willing to assume joint and several liability for Party B's insurance.

Unmarried pregnancies take remedial measures, each time a violation is paid ____ yuan, and remedial measures are enforced.

3. Party B fails to reach the late marriage age to pay the guarantee money, and until the late marriage age approves the birth, Party A will refund the deposit in full.

Fourth, other matters

1. This contract is signed by both parties, and is sent to the notary public for notarization. If one party fails to pay liquidated damages after breach of contract, the other party may apply for enforcement directly to the people's court after issuing a compulsory public certificate by the notary public. It may also pursue the liability for breach of contract through a supervisory procedure or litigation.
2. The liquidated damages collected by Party A shall be included in the management of the childbirth expenses outside the plan.
3. Matters not covered in this contract shall be implemented in accordance with the Regulations of the _______ Provincial Plan and Childbearing Regulations and relevant laws and regulations.
4. This contract is in the form of one copy, and each party and the notary office hold one copy.
