Lawyer Medical Mutual Assistance Agreement

Both parties to the agreement:
Party A: ______ City ______ Law Firm Party B: ______ City Lawyers Association provides a strong guarantee for lawyers in the event of accidents and major illnesses, reducing the worries of lawyers. Party A and Party B participate in the establishment of Party B's ______ The city’s lawyers’ medical mutual aid fund reached an agreement. In order to clarify the rights and obligations of both parties, the special agreement was as follows:

1. Party A participates in the ______ City Lawyer Medical Mutual Assistance Fund organized by Party B for one year. From ______ years ______ months ______ to ______ years ______ months ______ days.

II. Rights and Obligations of Both Parties Party A shall pay RMB ______ per person per year to participate in the mutual aid, and pay it in one lump sum every year in ______ months.
Party A participates in the mutual aid personnel, and the following circumstances occur during the mutual assistance validity period. Party B bears the corresponding payment responsibility. The specific criteria are as follows:
Accidental death pays RMB ______ yuan;
The death of the disease is paid in RMB ______ yuan;
One of seven major diseases, including malignant tumors, uremia, vital organ transplantation, quadriplegia, coronary artery bypass surgery, severe stroke, and myocardial infarction,

After the diagnosis, RMB ______ yuan will be paid.
The hospital will pay RMB ______ per day, and each person will not exceed ______ days per year.
Party A's participation in the mutual aid personnel will cause illness during the validity period of mutual assistance. Party B shall reimburse Party A for part of the medical expenses according to the following criteria:
a general emergency medical expenses:
Party B shall not bear the expenses of the account section, and Party A shall be responsible for it; however, Party A shall reimburse ______% of the ______% of the Class B drugs that the Party A personnel go to the pharmacy to purchase drugs or go to the hospital for medical treatment.
For the self-concessionary expenses, Party B shall bear ______% according to the actual concession of the participants of Party A;

For the above general emergency medical expenses, the maximum amount that Party B will assume for each Party A is RMB ______ yuan per year, the medical expenses for hospitalization or emergency room observation, and Party B is the maximum amount of Party A's participation in the merger. It is RMB ______ yuan.

Reimbursement, please bring the medical fee invoice and medical record card, and the designated person of the office will handle it.
This mutual assistance fund is only available to the participants themselves. It is not allowed to be borrowed or impersonated. Once found, the reimbursement qualification is cancelled.
1. Party A must participate in the mutual aid fund with all full-time lawyers and all full-time administrative staff. A full-time lawyer or full-time administrative staff refers to the original full-time staff who have retired from the Party A or from Party A. They do not include part-time lawyers and staff members who work as full-time lawyers after retiring from other units.
2. Party A's participation in mutual aid shall be attended by ___ City Urban Workers Basic Medical Insurance Party A. Participants must be hospitalized in the hospitals prescribed by the city.
3. Party A shall provide legal and valid credentials for applying for mutual assistance. It shall be claimed within _____ days of the relevant event or medical expenses, and the overdue shall be deemed as a waiver.
Seven major diseases refer to hospitals diagnosed within the mutual aid validity period. For example, major diseases that occur before participating in medical mutual assistance are not eligible for payment. During the period of mutual assistance, Party A’s participants shall leave Party A’s mutual assistance treatment according to the following reasons:
If you continue to work in the city's lawyer industry, your mutual assistance benefits may continue to be enjoyed until the expiration of this Agreement;
If you leave the city's lawyer industry, the mutual assistance treatment will no longer be enjoyed from the date of leaving Party A, and the mutual assistance money will not be returned.


3. The matters not covered by this Agreement and the objections arising from the performance of this Agreement shall be settled by both parties in consultation with the relevant provisions of social medical insurance and the characteristics of mutual assistance and mutual assistance of lawyers.
This Agreement shall become effective after the parties have signed and paid the full year of mutual assistance for participants. Upon expiration of this Agreement, if both parties have no objection, the validity period will automatically be extended for one year, and so on. However, Party A shall pay the relevant mutual assistance funds within the first month of the extension. Otherwise, the agreement is considered terminated.
