Party A and Party B agree to handle the slimming beauty contract in accordance with the following agreements:

The first definition of slimming beauty

The so-called slimming beauty of this contract refers to the non-medical behavior of comprehensive guidance measures implemented to maintain, improve the body and feel the bodybuilding by means of crafts, machines, utensils, materials, cosmetics, food, etc.

Slimming beauty projects include:

Body shape, weight control, adjustment;

Skin care;

Body oil pressure;

Facial beauty and makeup;

Hair removal

Beauty consultation and the sale of related products.

The basis of the second rights and obligations

The rights and obligations of both Party A and Party B regarding this slimming and beauty are in accordance with the provisions of this contract; if this contract is not agreed, it shall be stipulated by relevant laws and regulations.

The subsidiary files of this contract, the advertisements of Party B and the verbal agreements between the parties to this contract are all part of the contents of this contract.

Other special agreement matters between Party A and Party B are more effective than the terms of this contract.

Article 3 Contracting of minors

Party A should have full capacity.

If Party A is a person with limited capacity, he or she must obtain permission or recognition from his legal representative. This contract is valid.

If Party A is a person with incapacity, it shall be represented by his or her legal representative on behalf of him and on behalf of him.

Article 4 Description of the rights and obligations of members

Party A may apply to become a member of Party B if necessary, and its rights and obligations shall be in accordance with the provisions of the Member's Statute.

Party B shall make a clear explanation to Party A at the time of contracting regarding the rights and obligations of the member type and membership. If the membership card is lost, damaged or stolen inadvertently, Party B shall make a new card for free after Party A fills in the application form.

Article 5 Description of Courses and Affiliated Products

Party B shall accept Party A's conditions for slimming and beauty treatment, as well as the items, considerations, times, duration, number of courses, effect analysis, side effects and risks selected by Party A, as well as the necessary purchases for slimming and beauty treatment. The content, nature, utility and price of the goods shall be clearly and clearly stated to Party A at the time of contract, and relevant pictures shall be provided.

Party B shall keep a record of the services provided by Party A and sign it with Party A for confirmation and keep it for at least two years after the record for checking. Party A may request Party B to provide the aforementioned record of the record at any time.

Article 6: Inquiries and Disposal Obligations of the Operator

Before implementing the slimming beauty project, Party B should ask and confirm whether Party A is suffering from allergies, whether it is allergic, whether it is not taken, whether skin is sensitive or not, and other factors that are not conducive to slimming and beauty. Party A shall be honestly informed about Party B's inquiry.

When Party A accepts slimming and beauty, if any party finds that Party A's physical condition is abnormal or the location of the implementation is abnormal, it should immediately inform the other party. Party B shall immediately suspend the implementation and shall be obliged to take appropriate measures such as treatment and treatment by Party A. However, the reason for the abnormal or abnormal situation of Party A, if it is not the implementation behavior of Party B, Party A shall bear the relevant expenses for the measures taken by Party B.

Regarding the selection of doctors and physicians, Party A's opinions should be respected. During the period of treatment of Party A, the period of the slimming beauty contract should be extended.

Article 7 Charges

The cost of the slimming beauty course for this contract amounts to RMB ______ yuan; the total cost of supplies required for the course is RMB _________ yuan. See the attached file for details.

Article 8 Payment method

Party A may pay the consideration in full or in accordance with the progress of the course.

Article 9 Use of Cards

If Party B uses cards, vouchers or other similar means to provide services, it shall explain to Party A the use of cards, vouchers, service contents, time of use, place of use, number of uses and period of validity, etc. The project is stated on the card and coupon.

The above-mentioned effective period means _________ years from the scheduled start date of the slimming beauty service. However, in the future, if both parties have agreed to a deadline, this is not the case.

When terminating the contract or canceling the contract, Party B shall recover the card and the coupon. Regarding the standards for refunds and compensation, the cards and vouchers shall be handled in accordance with the provisions of Articles 10 to 13, 17 and 18 of this contract, in addition to the original price at the time of contract.

Article 10: Before the implementation of the contract, the consumer arbitrarily cancels the refund standard of the contract

If Party A terminates this contract before the implementation of the slimming beauty course, Party B shall refund the fees already charged to Party A within one month according to the following calculation standards. The refund amount is equal to the total amount of the deduction and the cancellation fee.

The cancellation fee of the preceding paragraph refers to _________ of the total amount of the contract price.

After the implementation of Article 11, the consumer arbitrarily terminates the refund standard of the contract

Party A shall terminate this contract after the implementation of the slimming beauty course. Party B shall refund the fees already charged to Party A within one month according to the following calculation standards. The refund amount is equal to the total amount of the price minus the fee for the service received, and the deducted amount of the attached goods is deducted, and the termination contract fee is deducted.

The termination fee for the termination of the preceding paragraph refers to the total amount of the price deducted from the service received and the _________ of the remaining amount of the opened goods.

The prices of the services received and the opened goods are subject to the original price at the time of the contract.

Article 12: Before the implementation, the compensation standard for the arbitrarily canceling the contract

If Party B terminates this contract before the implementation of the slimming beauty course, it shall be refunded within one month.

Party A has paid the fees and compensated Party A for the losses.

The loss of Party A in the preceding paragraph refers to _________ of the total amount of this contract. However, Party A can prove that the damage suffered by the above amount exceeds the above amount.

Article 13: The operator may cancel or terminate the contract

If Party B has one of the following reasons, it may terminate all or part of this contract after the slimming beauty course is released or implemented:

It is impossible to perform due to force majeure, such as natural disasters, wars, strikes, government decrees, etc., or irresponsible to Party B’s reasons.

The gender and age of Party A do not meet the conditions for Party B to implement slimming and beauty, and Party B is not aware of the fault at the time of contracting.

Party A is unable to complete the course of this contract due to illness or poor health.

In the above circumstances, Party B shall refund the fees to Party A in accordance with Article 10 or Article 11 of this contract. However, in the first case of the preceding paragraph, Party B shall not deduct the handling fee.

Article 14: Obligation to protect the industry after the termination of the contract

Party A shall terminate this contract after the implementation of the slimming beauty course. Party B shall be obliged to provide necessary information or assistance for Party A's life, body or health during the corresponding period.

Article 15 Ways of Dissolving a Contract or Terminating a Contract

Both Party A and Party B may express their intention to terminate the contract or terminate the contract in writing or verbally.

When the contract is terminated in writing or the contract is terminated, the style is referred to the attached file.

Article 16 Guarantee clause

Party B shall guarantee Party A's effect, and if it fails to achieve its agreed effect, Party B shall compensate Party A for RMB _________.

Article 17 Changes in Consumers

After Party A has agreed with Party B, Party A may transfer its rights and obligations under this contract to a third party.

The third party specified in the preceding paragraph shall bear all the rights and obligations of Party A in accordance with this contract from the time of the consent of Party B.

Article 18: Changes in the industry

After Party B has agreed with Party A, Party B may transfer the rights and obligations arising from this contract to other slimming and beauty companies.

In the case of the preceding paragraph, Party A may terminate or terminate the contract if it does not agree. Party B shall not deduct the handling fee upon refund. Party A may request compensation if it is otherwise damaged.

Article 19: Contracts are performed on their behalf

Party B may cancel or terminate the contract when Party B has delegated all or part of this contract to other slimming and beauty companies without the consent of Party A.

After Party B's consent, Party B shall be deemed to be the agent or user of Party B in respect of the performance of this contract.

In the case specified in the first paragraph, Party A may terminate or terminate the contract if it does not agree. Party B shall not deduct the handling fee upon refund. Party A may request compensation if it is otherwise damaged.

Article 20 Confidentiality obligations of the industry

Party B shall be kept confidential and shall not be improperly used if Party A participates in the slimming and beauty course of this contract and knows or holds the course items, course records and other relevant personal data of Party A.

If Party B violates the provisions of the preceding paragraph and causes Party A to be damaged, it shall be liable for compensation.

Article 21 Qualifications of employees

A beautician, dietitian or other practitioner who provides services under this contract must have legal professional qualifications.

Article 22 Choice of Service Spaces

Party A may accept slimming and beauty services at Party B's branches.

Article 23 After the contract is signed, both parties agree to change the contract.

After the contract is concluded, Party A and Party B may change the contents of the contract as agreed.

Article 24: The solution to the contract dispute

The disputes arising during the performance of this contract shall be settled by the parties through negotiation; or may be mediation by the relevant department; if the negotiation or mediation fails, the following _________ methods shall be adopted:
