According to the state's policy on personnel administrative relations and file management of mobile personnel, both parties have reached the following agreement on the principal and agency matters on a voluntary basis:

Party A’s responsibilities:

Party A provides Party B with the following _________ services:

1. Manage personnel and administrative relations and personnel files;

2. Responsible for the collection, identification, sorting, filing, and custody of personnel file materials. 3. Retain its original identity, calculate the length of service, and approve the file salary;

6. Issue various relevant documentary evidence based on marriage, maternity status, notarization, etc.;

7. Charges for the payment of social insurance fees and related procedures for retirement;

8. Manage the party organization relationship;

9. Handling personnel transfer and being responsible for the inspection and transfer of personnel files;

10. Accept various training applications, conduct employment guidance, and provide talent quality assessment;

11. Keep the personnel files in a simple manner.

Party B’s responsibilities:

1. Comply with the relevant provisions of the current personnel policy and cooperate with Party A to do a good job in management.

2. According to the _________ government _________ document, the personnel agency fee will be paid to Party A. The standard is _________ yuan/month. A one-time payment of personnel agency service fees for a limited period of this Agreement is required. If there is no payment within the time limit, Party A has the right to stop all personnel agency services.

3. Upon expiration of this Agreement, the renewal or transfer procedures shall be processed in a timely manner. In the past six months, Party A will seal the file of Party B and will not go through any formalities. This agreement is invalid.

4. Pay social insurance premiums and sign the "Agency for Social Insurance Payment Agreement".

5. Conscientiously abide by the "_________ Provincial Plan and Childbearing Regulations" and sign the "Project Fertility Contract" with Party A. Party B, who is not a _________ account, has the same household registration status. Upon expiration of the agreement, the personnel file will be transferred back to the original unit.

This agreement is signed for a minimum of one year. If the contract is terminated in advance during the agreement period, the personnel service fee paid shall not be refunded.

3. Married persons, if they need to have children, consciously go to the street office where the account is located to apply for the family planning index.

6. The fertility personnel of the Planning Department shall be referred to the street office of the family planning department or the household registration office in accordance with the provisions of Article 32 of the _________ Provincial Plan and Childbearing Regulations.

If the situation of Party B changes, Party A shall be notified in time and a new contract shall be signed. Otherwise, Party B shall be responsible for the consequences.

This contract is made in two copies. Each party and Party B shall hold one copy and have the same effect. It shall take effect after the signature and seal of both parties. Matters not covered in this contract shall be implemented in accordance with the provisions of the _________ Provincial Plan and Childbearing Regulations.
