Medical intermediary service agreement

Medical Technology Development Co., Ltd. is a professional company that provides medical information consultation and medical service intermediary for members. The two parties have reached the following agreements on service projects:

1. From the date of signing this Agreement, Party B shall become Party A's “Health Club Member” for a term of one year. From the date of the year to the day of the month, the service expires and the contract is automatically terminated.

2. Party B shall pay the membership fee of RMB in one lump sum, and the service target is.

3. Party A shall provide Party B with the following services within the time limit:

1) Conduct a comprehensive Western medical examination, psychological assessment, and health history survey for Party B

2) According to the inspection results, the experts will conduct health assessment for Party B and formulate a health plan.

3) Establish a detailed, comprehensive and dynamic “health file” for Party B

4) Instruct Party B to conduct “sub-health” conditioning

5) Provide Party B with health club member special hospital full-course medical services: emergency, outpatient, consultation, hospitalization, surgery

6) Arranging health consultation and health tracking for health care providers for Party B

7) Hold “Health Knowledge Lecture” and “Health Care Salon” in due course

8) Provide relevant medical information and health knowledge to Party B in due course

4. If there is any dispute in the process of cooperation, the two sides will conduct consultations in a sincere and friendly manner. If it cannot be settled through negotiation, either party has the right to submit the dispute to the Economic and Trade Arbitration Commission and conduct arbitration in accordance with the arbitration rules of the Association. The arbitral award is final and binding on both parties.
