Development history and social status

Labor and social security are the foundation of the people's livelihood and a sunrise industry that maintains social stability. Case 1: "One old and one small" major illness medical insurance policy

1. Policy origin

Defects of the system: In recent years, the medical security system of xx city has been continuously improved and its coverage has been continuously expanded. However, there are still quite a few groups including some college students, primary and middle school students, infants and young children, as well as some urban elderly people. The scope of protection, their medical treatment of medical problems has become a hot spot of social concern, especially the medical expenses of some seriously ill and seriously ill patients have brought a heavy burden to the family and society, affecting the process of building a harmonious society.

Basis of Laws and Regulations: On June 8, 2007, the municipal government issued the “Implementation Opinions on Establishing a Medical Insurance System for the Elderly and Children and Children with Major Diseases in the City of xx City”, which means that the city’s urban workers’ medical insurance system The transformation of the urban residents' medical insurance system will continue to improve the medical security system and the coverage will be further expanded.


2. Specific implementation methods

Payment standard:

The threshold for the reimbursement of medical insurance for the elderly in urban areas is 1,300 yuan, and the excess is reimbursed at a rate of 60%. The maximum amount of payment for a medical insurance year is 70,000 yuan.

Payment standard:

Financing standards for urban elderly medical insurance for the elderly: 1,400 yuan per person per year, of which 300 yuan for urban elderly individuals and 1,100 yuan for financial assistance.

Student and child illness medical insurance financing standards: 100 yuan per person per year, of which 50 yuan for individuals or families; 50 yuan for financial assistance.

Insured method:

The urban elderly are from January 1 to December 31 of each year as the major illness medical insurance year. From September 1st to November 30th of each year, the medical insurance premium for major illnesses will be paid in one lump sum according to the payment standard, and the medical insurance for major illnesses will be enjoyed from January 1 of the following year. Persons who meet the conditions for participation in the current year shall, within 90 days from the date of reaching the age of participation, apply for the payment of the old-age medical insurance for the elderly in the city where they live in the household registration book, and pay the medical insurance premium for the current year according to the payment standard. . From the next month after the insured payment, you will enjoy the medical insurance for the elderly in the town, and enjoy the treatment until December 31 of the current year.

The student and child are from the September 1st to the 31st of the following year as the major illness medical insurance year. All kinds of schools and child care institutions are responsible for the insured payment of the students and children's major medical insurance in the school. The students and children in the schools and child care institutions pay the one-time medical treatment according to the payment standard before July 1st to September 30th. insurance. Seek medical treatment and settlement:

3. Policy characteristics

This major illness medical insurance has five characteristics:

4. Results after implementation

Incorporating the "one old and one small" population into the social security system is an important measure for the government to improve people's livelihood. In order to ensure the smooth participation of “one old and one small”, the Labor and Social Security Bureau of xx City xx District has taken a number of measures to make “one old and one small” achieve outstanding results:

By the end of 2007, xx areas had participated in 163 medical insurance units for the elderly and small and large, and the number of participants was 66,055. 5. People's views

Since the implementation of the “one old and one small” major illness medical insurance policy, most of the elderly, children and students within the scope of their participation have actively participated in the insurance. The scope of policy influence has been continuously expanded, the number of beneficiaries has been increasing, and the social security system for people's livelihood has been continuously improved and promoted. The harmony and stability of the entire society has provided a solid backing for economic development.

However, the more developed, the easier it is to find problems, especially the beneficiaries who are at the forefront of the policy are most likely to find the system's shortcomings and problems in the implementation process. To this end, the group conducted interviews with beneficiaries. The so-called "the eyes of the people are sharp", therefore, the opinions of the people are also worthy of attention:

First of all, when most people talk about this policy, they think that the government can have such a move that it attaches great importance to the people's livelihood issue, but it does not mean that the policy is perfect. They believe that the medical expenses reimbursement conditions involved in the “one old and one small” medical insurance policy are too restrictive. For example, the equipment expenses that account for a large proportion of medical expenses during the treatment of major illnesses are not included in the scope of reimbursement. The fees that can actually be reimbursed are often only a few, and most of the expenses have to be borne by the people themselves. Of course, this is not entirely a matter of the policy system. The country's economic level is still underdeveloped. To develop, we must not only consider fairness and disregard efficiency. It is hoped that in the future development process, social security can be developed in parallel with the economy.

Second, most people have such a view. In the process of their medical treatment and treatment, the hospital side often uses the people's lack of understanding of medical knowledge to make redundant medications and inspection items. The hospital believes that the public has medical insurance, so it will not increase the burden. We believe that such ideas and practices are extremely bad. This is not only a plunder of state property, but also a great waste of social resources. Today, when the economy is urgently demanding and developing fast, both of these situations are Should not appear. The group believes that this phenomenon is caused by the imperfection of the national supervision system and the lack of public knowledge. Therefore, accelerating the improvement of the state's institutional system in this respect and increasing the propaganda of science, education, culture and health are undoubtedly the top priorities for solving this problem.


Case 2: “Non-Social Security for Old-age Residents”

1. The meaning of "no social security for the elderly"

Older people aged 60 or older in urban or rural areas and who do not have any of the following conditions are classified as “socially inhabited elderly residents without social security”:

Enjoy the retired fees, retirement fees, and retirement fees of institutions and institutions;

Retirement retirement reimbursement fees and living allowances for retired personnel issued by the Social Insurance Co-ordination Fund;

Enjoy the disability allowance for workers in the first to fourth grades of work-related injuries issued by the Work Injury Insurance Fund, and the public and the detainees shall provide pensions for immediate family members;

Enjoy the living subsidies for the construction of land acquisition and overtime personnel;

Enjoy the retired gold of the self-employed military cadres, hand over the local management of military cadres, and no pension for military employees;

Enjoy rural social pension insurance benefits;

Continue to pay the basic pension insurance premium.

2. The origin of the policy

Social reasons: In order to further improve the social security system, guarantee the basic living of the elderly residents, and achieve the goal of social development of “the old and the young”, according to the “Measures for the old-age security for the elderly in urban and rural areas without social security”, xx City xx District Labor and The Social Security Administration has stepped up its implementation of the policy of “no social security for the elderly to support the elderly”.

The actual reason: the survey data of the urban and rural insecure elderly residents shows that the urban residents in the xx area are over 60 years old and the female is over 50 years old, including 634 people over 80 years old, 7056 years old from 70 to 80 years old, 70 years old. 3. Specific implementation plan

The regional social security department made people aware of the policy by issuing brochures, and people participated in this social security activity by means of voluntary declaration.

This policy will be implemented from January 1, 2008. Older people who meet the conditions of non-social security for the elderly can receive the “Application Form for the Elderly in the Urban and Rural Non-Social Security for the Elderly” in the local social security institution. The information will be submitted to the local social security agency for review. The application form that meets the requirements will generally receive a monthly subsidy of RMB 200 after 20 working days.

On the 20th of each month, the municipal labor and social security department will use the financial institutions to put the welfare pension into the old man's account on time. It should be noted that the elderly who apply after January 2008 can also receive the security from January to the current month.


4. Impact after implementation

In our group's visit to Hancunhe Township in District xx, we can find that the policy of “protection of old-age residents without social security” has a strong response in the local area. People actively participate in the declaration activities of this policy. . As of the end of March 2008, Hancunhe Town, which was inspected, reached 100% of eligible elderly applicants. This reflects people’s urgent desire for a sound social security system and the enthusiasm of local people to participate in social security. Of course, this is also beneficial to the local public security agency's vigorous publicity.

However, it is not a good policy that will satisfy the public. The best policy must be ensured by a highly transparent implementation program and an efficient execution team to achieve the desired results, thus truly benefiting the public. Let the people be satisfied. Through interviews with local people, we learned that most people are very concerned about social security and are willing to supervise social security institutions. The introduction of this new old-age social security system not only makes the people feel the benefits of economic development, but also feels the government's concern for the elderly. Here, the ruling organs at all levels are reminded to pay attention to the supervision and opinions of the people and to pay attention to the transparency of governance.

Of course, although the policy that has just been introduced has been recognized by most people, it will have its own shortcomings. At present, the biggest shortcoming is that the elderly people who are benefiting are not wide-ranging, and there are still many restrictions. This is inseparable from the fact that China is still in the middle stage of development. I hope that in the future economic development, the government can pay more attention to the combination of fairness and efficiency, expand the scope of beneficiaries, and truly realize the "oldhood" of our society. .


Case 3: Helping measures for “zero-employment families”

1. The main cause of unemployment

Unemployment insurance is an important part of the social security system and one of the main items of social insurance. From the current situation in China, the unemployment rate is still at a relatively high level, and the main causes of unemployment are as follows:

Labor supply exceeds demand. China is the world's most populous country and a developing country. The level of economic development is relatively low, and other economic resources are relatively short, which restricts the development and utilization of labor resources.

China is making major adjustments to its economic structure, and the corresponding adjustment of the labor structure must inevitably lead to the loss of some people. This structural unemployment has increased unemployment pressure.

Along with the advancement of science and technology and the improvement of labor productivity, some areas, especially the traditional sectors of the primary and secondary industries, can not only expand the employment capacity, but will reduce the number of employed people and divert some labor, resulting in an increase in the number of unemployed people.

Because many unemployed people have a single skill, the level of professional skills is not high, it is difficult to suit the needs of employers, and the concept of career choice is outdated. It is impossible to rely on their own efforts to open up employment opportunities and increase the difficulty of realizing re-employment.

China's current social security system is imperfect and its coverage is narrow. The market employment mechanism has not yet been fully established, and it also has obvious restrictive effects on labor mobility and rational allocation.


2. Specific measures for the "zero employment" policy

The economic ability of a family is directly related to employment. The existence of unemployed people is not only a waste of their own resources, but also a drag on the family economy. For a “zero-employment family” with zero family employment, life is even more difficult. Employment of the zero-employment family is very difficult due to age, physical condition, low cultural skills, and unsuitable employment concepts.

In order to help these families get out of the difficulties, the Labor and Social Security Bureau of xx City xx District has adopted the goal of creating a full employment zone in order to achieve the goal of “zero employment families” in the region, and adopted different measures for the specific situation of “zero employment families”. Help measures:

Give priority to the employment of zero-employment family laborers with urgent employment aspirations, so that they can be employed as soon as possible;

3. Results after implementation

The results of the above policies are gratifying, as can be seen from the specific data:

Through the implementation of the “zero-employment family” assistance policy and other series of employment-promoting measures supplemented by other employment-solving policies, xx districts achieved 14,107 new employment in 2007, completing 102.6% of the city's annual target. , an increase of 124 people over the same period of last year; 3740 people were employed in employment difficulties throughout the year, completing 138.52% of the city's annual target, an increase of 1020 people; the identified 726 “zero-employment families” were fully cleared before June 25, and were overfulfilled. 4. Analysis of advantages and disadvantages

advantage:

Disadvantages:

The “zero-employment family” assistance measures include training, guidance and introduction in various aspects. There are many trivial tasks involved, which requires more people. It is easy for social security institutions to ignore other employment work. During our exchanges with the Hancun River people, we learned that local social security institutions have neglected the unemployed in non-zero-employment families, and this group of employees is second only to the “zero-employment family”.


Problem

Government consciousness
The xx area is the old industrial base of xx city. The population is aging and the speed is large. The huge population is facing more and more pressure on the needs of the old-age, medical and social services in the district. The total scale of pension expenditure, retirement The expenditure on medical expenses for personnel has been increasing year by year, which has brought a certain burden to economic development.

A large number of non-public, individual enterprise workers are not covered. The inspection found that the current social insurance coverage in the district is narrow, and the phenomenon of unpaid insurance premiums is serious. At present, a large number of non-public enterprises and individual enterprise workers are not insured. Most migrant workers are difficult to participate in the insurance according to the current system, and the “temporary workers” employed by administrative institutions have lower participation rates.

Retirement workers in the old-age payment work have increased significantly, and the treatment has also been significantly improved. However, the survival problem of retired workers has not been fundamentally solved, and the loopholes cannot be blocked from the source. The problem of “frauding” still exists.

It is difficult to expand social insurance, and some enterprises have difficulties in operating conditions. Some unit leaders and some workers are less conscious of their participation in insurance. Unit leaders are unwilling to participate in insurance, and employees themselves do not want to participate in insurance.

Social security has fewer personnel and heavy tasks in individual departments, and the existing working environment and office conditions are insufficient. For example, there are 5 social inspection departments of the xx Labor and Social Security Bureau. According to the "Social Insurance Auditing Measures", more than two auditors are required to conduct audit inspections. At present, there are 5 personnel in the audit department, and 500 enterprises are audited on an annual basis. It takes at least 5 or 6 years to calculate the units that have already been insured, and it is not enough to handle complaints, report cases and calls from arrears.

With the promulgation and implementation of the Labor Contract Law, under the current situation of adjustment of labor relations and increasingly heavy work tasks, many potential contradictions have arisen and intensified, and the promotion and implementation of labor and social security policies have become more difficult.

The public’s call for public medical trials has made the individual burden heavy and the reimbursement standards are not uniform. As the current policy of medical insurance can not find the corresponding legal basis in many rules, the general hospitals are generally confused with the collection fees, and a large part of the fees collected are not within the scope of reimbursement, resulting in the policy of reducing the burden on the people. The role of the party has been greatly reduced. The problem of "expensive medical treatment and difficult medical treatment" has not been fundamentally resolved.

The new rural pension insurance policy is not strong enough, especially the government's funds for farmers' subsidies are too small, and the attraction is not great, and the insurance payment threshold is high, and there is no collective subsidy. It is expected that the pension will not be received. It is not worthwhile.

In the grassroots departments, there are still phenomena of “having policies and countermeasures”. Some of the staff members are not serious and irresponsible for their own work. What is more, they have to exploit the policy, to bully them, to fill their pockets, and to harm the national collective interests. . For example, when the rural cooperative medical policy is introduced, farmers can voluntarily choose the standard of participation, but some cadres are reported according to the highest standards, and the illegal activities of fraudulent protection from them have a very negative impact on the people.

Our discovery of the Labor and Social Security Bureau lacks in-depth, meticulous and comprehensive guidance and mobilization in the propaganda work. During the visit, we found that most towns and village residents did not have a deep and comprehensive understanding and understanding of the policies introduced by the labor and social security undertakings in the region, and some even knew nothing about them. Some people said that if the state introduces a policy, we will pay if we pay the money. Others have no ability and do not want to ask questions. Some people have formed feelings about these policies and even expressed opinions on policies and the government. On the one hand, this has added many difficulties to the implementation of new policies and new regulations. On the other hand, after residents participate in insurance, they cannot effectively use laws and regulations to protect their legitimate rights and interests. This is contrary to the original intention of the policy.

At the time of work, supervision and inspection were not enough. When problems were found, more corrections were made on the spot, and less serious treatment was handled. After some systems were issued, the effect of implementation was not checked in time, which reduced the enthusiasm of government officials to serve the people and affected The efficiency of work has caused some illegal elements to take advantage of the law and harm the interests of the country and the people, and at the same time have had adverse effects among the people. During the visit, some residents reported that there is still a phenomenon of corruption and deception in the social security work in the region. It has not been effectively stopped and has had a negative impact on society.

The construction and development of social security institutions are not balanced, and the contradiction between the infrastructure construction of social security institutions and the needs of service targets still exists. Still one of the 25 social security agencies in the area did not meet the target. In the course of the field investigation, we found that the infrastructure and service environment of Hancunhe Social Security Office still has a certain gap compared with other districts and counties. Hancunhe Social Security Office has a total of 5 staff members. The working space is small and the equipment is backward. In just two hours of interviewing with us, there are dozens of various insured personnel to handle related business, and the staff are busy. Through these, on the one hand, we can see that the social security work in the region is in full swing, and the results are satisfactory. On the other hand, we can also see that the direct investment and attention of the government is far from enough. How is the social security system of this scale? Can meet the social security needs of the people of the town. This should arouse the government's attention and deep thought, and should provide sufficient support from all aspects of hardware, software and policy.
