1. The service building has been expanded, and the software construction has been more standardized. Last year, the center was equipped with a community health service platform, and the clinics in the center were also equipped with computers. This makes the health record in the community health service platform a dead file. In order to solve this problem, the center overcame various difficulties. In April this year, the regional network was finally established, which greatly improved the utilization rate of the community health service platform, and laid a good foundation for fundamentally solving the problem of dead files.
On this basis, the center has established twenty-eight types of rules and regulations, namely, professional ethics and codes of conduct for all types of personnel, job responsibilities of various personnel, training, management, assessment and reward and punishment systems for various personnel, health education. The software construction of the center, such as prevention, health care, medical care, rehabilitation, and technical guidance for fertility planning, etc., the construction of the software of the center is more perfect.
2, health education is often unremitting, community prevention and comprehensive development of the center's health education work at the beginning of the year to develop a plan. The Center regards the popularization of health knowledge as the focus of the year's work. Community residents' health awareness rate is 75%.
The Center timely registered and reported 3 cases of legal infectious diseases; assisted the city and district CDC to carry out disease surveillance, epidemic investigation, infectious disease source investigation and death cause investigation; and actively carried out prevention and treatment of common infectious diseases. At the same time, the center also supervised the diagnosis of confirmed tuberculosis, and the individual case rate was 90%.
The Center has overcome many difficulties and improved the working conditions of the vaccination room. The passive situation of child immunization prevention has been greatly improved.
The Center has initially mastered the population data of the population under 7 years old. The community vaccination certification rate of the three communities directly managed by the Center reached 100%. The standard vaccination rate of the six vaccinations for routine vaccination reached 95.1%.
As of June 30, 20nn, the Center and its affiliated Fengxiang, Recreation, Dongguan, Zebei, and Nanjie Community Health Service Stations established 23,498 family health files and personal health records for residents of Dongjie Community. Up to 55.8%.

In the past six months, the center has conducted health checkups for 2048 cadres and workers of community residents and some administrative units, institutions, factories and mines, schools, etc., and has been well received by the general public. 4. The center carried out prenatal and postnatal care, contraceptive knowledge guidance, and counseling services; freely distributed contraceptives for community residents; using households to establish health records and follow-up opportunities, educated women of childbearing age on family planning, prenatal and postnatal education; Provided counseling and guidance for family planning. The technical guidance work for the whole family planning is step by step. Project fertility, prenatal and postnatal care have gradually become the conscious actions of residents.
The Center and various community health service stations registered persons with disabilities in their jurisdiction. There are 265 disabled people, of whom 121 are physically disabled, accounting for 45.7%; 44 are mentally disabled, accounting for 16.6%; 40 are visually impaired, accounting for 15.0%, and 23 are intellectually disabled, accounting for 8.7%; 12 people, accounting for 4.5%; 11 people with speech disabilities, accounting for 4.2%; 14 other people with disabilities, accounting for 5.3%. 5. The grassroots management has been carried out in an orderly manner. Through practice, we have three experiences:

Establish a volunteer team. In the community health service work, a reliable volunteer team must be established. In addition to community health services, community-based physicians and individual practitioners in the community are an important force to rely on.

Second, the problem
1. With the establishment of community health service institutions, the public welfare of community health service institutions needs to be further established and reflected;
2. The community health service team is far from being able to adapt to the needs of the rapid development situation, the number needs to be increased, and the quality needs to be improved;
3. Staffing, staff salaries and office expenses of community health service agencies need to be implemented as soon as possible;
4. The degree of informatization of community health service institutions needs to be further improved. The problem of lack of transportation needs to be resolved as soon as possible.
Third, intend
1. Continue to give full play to the advantages of experts, and unswervingly follow the path of experts into the community, so that more residents can enjoy high-quality medical and health services at their doorsteps. Continue to run a special specialty, so that advanced prostatectomy and minimally invasive disc technology bring the gospel to more patients;
2. Pay close attention to the construction of service concept and implement three transformations, that is, the transition from treatment-based to prevention-based, from patient-centered to health-centered, from waiting for patients to active home-based services;

4. Continue to build health records for residents, and the filing rate should reach 80%. Strengthen the screening and management of key chronic diseases. The blood pressure rate of the first diagnosis of patients over 35 years old is ≥90%, and the rate of hypertension is 80%.
5.
